If you are enrolled in an UnitedHealthcare plan and participate in the Sweat Equity program, you may be eligible to receive up to $200 reimbursement twice per year if you:
- Are an active member of an eligible UnitedHealthcare plan
- Use the gym for cardio workouts or take qualifying fitness classes 50 times in 6-months
